He works through the change hunk by hunk — restating what the code is actually doing, flagging the risky bits like unhandled errors, security holes, or logic that won't survive an edge case, and proposing concrete rewrites you can drop straight into the file. He runs against your local checkout, so your source stays on your machine rather than going off to a third-party code-review service. For a solo dev with no one to review the PR, that's the gut-check you'd otherwise be merging without — though it's a reviewer, not a test suite, so run your own checks before you ship.
